Oman- Badr al Samaa facilitates alternative to traditional open thyroidectomy

An expert in transaxillary endoscopic thyroidectomy, Dr Chandran adopts different approaches to thyroid disorders. In endoscopic thyroidectomy, thyroid is approached from the lateral aspect either via axilla or lateral chest in contrary to the conventional anterior open approach.

The surgical treatment of thyroid disease typically involves a transverse incision made in a highly visible area of the anterior lower neck. This open approach permits direct exposure and safe dissection of the thyroid gland for most diseases requiring surgical intervention. However, for some patients of darker-skin ethnic origin such an incision can result in hyperplastic or keloid scar formation within a highly visible area of the neck.

With the evolution of endoscopy and minimally invasive surgery, more and more doctors are into this field because of its advantages and patient acceptance.

The advantages of surgery performed using minimally invasive technique are enhanced cosmesis, optical enhancement, and improved visualisation through video magnification. All these have inspired the use of an endoscopic approach to the thyroid and parathyroid glands.

Dr Huscher in 1997 successfully applied minimally invasive thyroid surgery for benign
thyroid tumour. Ohgami M introduced endoscopic thyroidectomy using the breast approach.

'Now, several reports have described the feasibility and the better cosmetic results of endoscopic thyroidectomy than those of conventional open surgery,' said Dr Chandran.
