New Rehab Discharge Group in Palm Beach County

Paradise Home Health Care of Boca Raton Florida begins facilitating a new discharge group at the Rehabilitation Center in Barrington Terrace a Five Star Senior Living residence in Boynton Beach on February 20th 2015. The in-house Discharge Group will meet the 1st and 3rd Fridays of each month at 2:30 in the afternoon. The group is designed for both patients and their family members.

“The ultimate goal of rehab is returning home” explains Adrienne Tilley Skilled Admissions Director. “It is important for patients to learn important strategies for a successful transition and get answers to their questions and having family members there also is very exciting” adds Shirley Dodds Director of Social Services.
Topics covered include: Achieving best possible outcomes home safety & fall prevention avoiding preventable re-hospitalizations community resources and government benefits as well as what to expect from Medicare home health and choosing private care once home.

The discharge group is facilitated by Sherry Picker MSW a Social Worker with Paradise Home Health Care and Pam Hapke of Director of the Easy Living Program. “Rehab gets you “home ready” but not fully back to yourself. Many folks have lots of questions about what to expect at home and what services they are entitled to or may wish to hire. From considering grab bars an emergency response button or help with personal and household tasks we are able to address those questions and many more within a group setting” says Ms. Picker.

“From the private duty home care provider perspective it has always mystified me how in various hospitals and rehabs the patient seems unprepared to follow through on some basics about their discharge home – they are so occupied with “going home”. For example just knowing how they are getting home is often unclear. Also knowing to make a follow up appointment with their primary physician within the first week home is very important as well as understanding their medications and that Medicare home health will likely follow up with them with additional services” relates Linda LeMonte Administrator of Paradise Home Health Care.
