Vaginismus Clinical Trial Shows Strong Promise

A recent study by Peter T. Pacik MD FACS shows a 90+% success rate in treating vaginismus a condition which many health care providers fail to recognize yet effects between 1 and 7 percent of all women worldwide. This multimodal approach for the treatment of vaginismus expands on a prior FDA approval for research in this field. Treatment utilizes a combination of BOTOX® injections and progressive dilation under anesthesia as well as post procedure supervised dilation and sex counseling.

Vaginismus is a pelvic pain/penetration disorder marked by involuntary spasms of the vaginal muscles often brought on by the fear of penetration. As a result any attempt at sexual intercourse use of tampons or routine gynecological exam can be painful or impossible.

Peter Pacik MD FACS recently published his larger series of vaginismus treatment published in the International Urogynecology Journal with a continued high success rate minimal side effects and rare recurrence. A single treatment is all that is normally needed; Botox injections rarely need to be repeated once a patient is dilating and having intercourse.

This 2014 published report “Understanding and Treating Vaginismus-a Multimodal Approach. Clinical Opinion” is based on lessons learned in the treatment of more than 250 vaginismus patients and evaluation of more than 400 inquiries and was written to make vaginismus more widely understood to aid in the differential diagnosis of sexual pain suggest a variety of effective treatments and explain how Botox can be used as part of a multimodal treatment program to treat vaginismus.  With greater awareness among clinicians it is hoped that medical schools residency programs and medical meetings will begin teaching the understanding and treatment of Vaginismus. This report follows the previously published FDA approved research: “Open Label Single Center Pilot Study of the Use of BOTOX Injections Sensorcaine Injections and Progressive Dilation Under Anesthesia for the Treatment of Primary Vaginismus” which outlines Dr. Pacik’s groundbreaking treatment approach by using targeted BOTOX® injections intravaginally administered under anesthesia followed by additional injections of a long acting local anesthetic bupivacaine. Progressive dilation under anesthesia allows the patient to wake up in recovery with the dilator in place and no discomfort.

“The BOTOX® vaginismus treatment program continues to have high levels of success helping to relax and stretch the vaginal muscles which interfere with the ability to consummate relationships” states Dr. Pacik about his treatment program. The program addresses both the physical spasm noted in women suffering from vaginismus as well as the psychologic aspects of fear and anxiety to penetration so prevalent in this population.”

“BOTOX® alone without the full program including post-procedure counseling and follow-up is a setup for failure as has been seen in several patients” explains Dr. Pacik about the treatment. “Some of these patients had two or more courses of BOTOX® and failed each time.”

This method of treating Vaginismus has been used by Dr. Pacik since 2005 in more than 300 patients treated with a success rate in excess of 90%.

The FDA first approved Dr. Pacik’s research pilot study to treat Vaginismus in 2010 using a multimodal treatment program including BOTOX® and since then he has presented his research findings at numerous medical meetings including a podium presentation to the 2013 annual meeting of the American Urogynecologic Society conference in Las Vegas.
